Kedah Health Dept apologises after woman gets wrong second vaccine

The Kedah Health Department has apologised to a Covid-19 vaccine recipient and her family after she was mistakenly given the wrong type of vaccine for her second dose at a vaccination centre in Sik on Aug 5.

Its director Dr Othman Warijo said based on investigations, the mistake occurred after a nurse at the vaccination centre failed to check the details on the woman's vaccination card.
